Audiogram Policy

Each new athlete is required to submit an audiogram using ICSD's Audiogram Form. Athletes must have a hearing loss greater than 55 dB in the better ear.

To complete your audiogram, go to http://www.deaflympics.com/audiogramform.php, and enter your information to download the standard ICSD Audiogram Form.

Bring this form to a professional credentialed audiologist to complete and sign.

***Audiograms are due to USADSF at least four months prior to the start of the event.***

Please Note: The ICSD charges late fees for audiograms:

• Within 3 months of the event: $40

• Within 1 month of the event: $100
